- Original bank reference letter stating account details.
- Photocopy of family card.
- Printout of ticket reservation.
- Sponsorship letter in English on company letterhead (if applicable), stating sponsored individuals’ details.
- For Indonesian citizens:
– Photocopy of NPWP and SIUP
– Letter on plain paper with shop stamp, NPWP, and SIUP photocopies. - Other supported documents
