- Application letter (Surat Permohonan dan Jaminan)
- Scanned color copy of foreigner’s passport with minimum 12 months validity of passport (the identity page)
- Personal bank account with minimum amount of USD 5,000 or equivalent for at least 3 months period
- Latest color photograph with red or white background size 4X6
- Address in Indonesia
- Information/invitations/correspondence from government agency or private institution explaining the relationship with applicants.
- Other documents if required.
